You can download our claim form using the button below. Sections 1, 2, 3 and 4 of the claim form must be completed by you in full.

You can either print the claim form or fill it in digitally and save it using software such as Adobe Acrobat. Once these sections are complete, please pass the form on to your vet practice. Ask your vet practice to complete sections 5, 6 and 7. They will then forward on the claim form, their invoice and the clinical notes for the treatment carried out, along with any other relevant documents to us either by post or to claims@exoticdirect.co.uk.
